Early Intervention Services

Early Intervention Services includes outpatient group and individual counseling and group intervention for young same gender loving men. This intervention focuses on individuals at risk for substance use and HIV transmission.

Emergency Utility and Rental Assistance for Special Needs Populations

Emergency Utility and Rental Assistance for Special Needs Populations is provided by the funding from Broadway Cares for AIDS to help eligible needy clients with emergency fi nancial assistance to pay utilities or rent.

Florida AIDS Drug Assistance Program (ADAP)

Florida AIDS Drug Assistance Program helps eligible individuals who have health insurance to pay premiums and insurance co-payments to keep their private insurance.

Florida AIDS Insurance Continuation Program (AICP)

Florida AIDS Insurance Continuation Program helps eligible clients who have health insurance to pay premiums and insurance co-payments to keep their private insurance.

Sliding Scale Payment for Service

Empower “U”, Inc. Community Health Center is not a free clinic. Services are offered to clients regardless of their inability to pay. Fees are based on a sliding scale. This means client who are uninsured or those with a high deductible may be eligible for a discounted fee for services, co-payments and deductibles. Medical will be based on your gross household income and household size and is calculated using federal guidelines. To qualify, your total household income must fall at or below 200% of the Federal Poverty Level.

You may apply for discounted fee by completing and submitting the Sliding Fee Discount Form at the registration desk with the required proof of income and family size at your first appointment and annually thereafter. The following documents are acceptable proof of income:

• One month of consecutive pay stubs or letter of salary


• 1040 Tax Form


• Unemployment letter


• Social Security award letter


• Support letter, also called ‘Verification of No Income’ (for
those with no income) – This letter must be notarized

Billing Statements and Balances

EUCHC collects co-pays and fee-scale fees before your visit. We will bill your insurance for covered services. However, we will send you a bill if your insurance indicates that you are financially responsible for additional costs from your visit such as co-insurance, deductibles and non-covered services. Housing Opportunity for Persons with AIDS (HOPWA)

HOPWA is a housing program that assists eligible individuals who have HIV/AIDS to secure and maintain housing stability.

Navigators and Certified Application Counselors (Obama Care)

Empower “U”, Inc. Community Health Center trained Navigator and Certified Application Counselors assist clients with affordable care act insurance enrollment, provide information about insurance plans on the exchange, assist with enrollment in Medicaid, access florida food stamp applications and benefit programs for low income individuals. Our counselors includes Case Managers, Peers and Linkage to Care Coordinators.

Silver Saver Prescription Drug Program

Silver Saver Prescription Drug Program is a statewide program for Floridian’s age 65 and over who are enrolled in Medicare. The Silver Saver program can provide up to $160 per/month in medication cost.
