MEDICAL CASE MANAGEMENT

Medical Case Management Services include referrals, low income eligibility and a wide variety of support services in the community to help assist clients with meeting their needs. Medical Case Management plays an essential role in assisting clients with developing and adhering to a health care plan.

PRE-EXPOSURE PROPHYLAXIS (PREP)

Empower “U”, Inc. Community Health Center provides PrEP as a prevention option to reduce the risk of sexually acquired HIV infection in HIV negative adults at high risk. The goal of PrEP is to prevent HIV infection if you are exposed to the virus. If you are at ongoing risk for HIV exposure, speak to your doctor about PrEP.

CONDOM DISTRIBUTION

Empower “U”, Inc. Community Health Center is an official condom distribution center for The Florida Department of Health. Approved organizations and individuals can pick-up free condoms at our location. Please sign up with the Miami-Dade County Health Department to become an approved condom site. It is important to increase condom availability to reduce STI transmission like chlamydia, syphilis, gonorrhea and HIV in the community.

EARLY INTERVENION SERVICES AND INTERVENTIONS

Early Intervention Services include outpatient group and individual counseling and group interventions for young same gender loving men. Interventions focus on individuals at risk for substance use and HIV transmission.

EMERGENCY UTILITY ASSISTANCE

Emergency Utility Assistance is provided by funding from Broadway Cares, Equity Fights AIDS (BCEFA) to help eligible needy patients with emergency financial assistance to pay utilities.

FLORIDA AIDS DRUG ASSISTANCE PROGRAM (ADAP)

ADAP helps eligible patients with HIV to get life saving medication and can help to pay health insurance premiums and insurance copayments.

MIAMI TRANSPORTATION DISADVANTAGED PROGRAM

Daily, Weekly, and Monthly passes available for patients who meet low income eligibility requirements.

HOUSING OPPROTUNITIES FOR PERSONS WITH AIDS (HOPWA)

HOPWA is a housing program that assists eligible individuals who have HIV and an AIDS diagnosis to secure and maintain housing stability. Availability is contingent on funding and placement on the wait list. Waitlist placement is managed by the City of Miami Department of Community Development HOPWA Program.

NAVIGATORS AND CERTIFIED APPLICATION COUNSELORS (OBAMA CARE)

EUCHC trained Navigators and Certified Application Counselors assist patients with health care exchange insurance enrollment. We provide information about insurance plans on the government exchange, assist with enrollment in Medicaid, Access Florida food stamp program and other benefit programs for low income individuals.
